About COVID-19 Testing at Monarch
We use a rapid antigen test to detect active COVID-19 infection. The sample is collected with a gentle nasal swab from the front of the nose — no deep nasopharyngeal swab required. Most patients describe it as a brief tickle, not painful.
Results are available during your visit. A Monarch physician reviews your results with you and discusses next steps based on your symptoms, exposure, and any documentation you need.

If You Need a PCR Test
Monarch primarily offers rapid antigen COVID-19 testing. If your employer, school, airline, or destination requires PCR, call us before your visit so we can confirm whether send-out testing is available that day or direct you to the right testing site.

How COVID testing pricing works: The rapid antigen test fee is added to a brief office visit fee for symptom evaluation. We review the expected total with you before testing begins. Self-pay rates listed below; insurance billed at contracted rates.

We are in network with Anthem BCBS, United Healthcare, Aetna, Cigna, and Encore/IU Health Plans. We are NOT in network with Medicaid or Humana. Your copay, deductible, and coinsurance depend on your plan — we recommend confirming with your insurance ahead of your visit.

Documentation for Travel, Work, or School
If you need test results in writing, we provide same-day documentation. Tell the front desk what you need when you check in so we can prepare the correct form.
For Travel
We can provide a printed or MyChart-accessible result with patient name, test date, test type, and clinic information. If a physician signature is required, tell us before testing. Confirm with your airline, cruise line, employer, or destination country which test type and timing window they require before your visit.
For Return-to-Work
Negative test result and return-to-work clearance note for your employer. We can also document isolation timeline if you tested positive elsewhere.
For School or Childcare
Negative test note for return-to-school clearance. We follow current CDC and Indiana State Department of Health guidelines for isolation periods.
If You Test Positive
A positive rapid antigen test means active COVID-19 infection. A Monarch physician will discuss your symptoms, recommend treatment if appropriate (such as Paxlovid for high-risk patients), and give you guidance on isolation and when to follow up.
State reporting: Positive COVID-19 results are reported as required by Indiana public health reporting rules. Negative rapid antigen results may not require the same reporting pathway. Our team documents your result in your chart and can provide printed or MyChart-accessible documentation when available.
If you have severe symptoms — severe shortness of breath, persistent chest pain, confusion, bluish lips or face, or trouble staying awake — call 911 or go to the nearest emergency room. COVID-19 can become serious quickly in some patients.

COVID-19 Testing FAQ
Do I need an appointment?
No. Walk in any time during open hours: Monday through Friday 8am to 6pm, Saturday and Sunday 9am to 12pm. You can also book online through MyChart if you prefer to hold a time.
How long does it take to get results?
Rapid antigen results are available during your visit, usually within 15 minutes of the swab. If PCR send-out testing is needed, call us before your visit and we will let you know the typical turnaround time.
How accurate is the rapid antigen test?
Rapid antigen tests are useful for detecting active COVID-19 infection, especially when symptoms are present. A positive result is generally reliable. A negative result does not always rule out COVID-19, especially early after exposure or when symptoms are mild. A Monarch physician will review your symptoms, exposure history, and timing to help decide whether repeat testing or PCR testing makes sense.
Can my child get tested?
Yes. We test children. The front-of-the-nose swab is gentle and is well tolerated by most pediatric patients. For very young infants or severe symptoms, a Monarch physician may recommend pediatric emergency care depending on the situation.
Can I get a test result document for travel or work?
Yes. Same-day signed documentation is available. For international travel, confirm with your airline or destination which test type and timing window they require before your visit.
