WHY THE SIGNATURE MATTERS
Your School Form Gets Signed By a Physician.
Clinical placement coordinators review a lot of paperwork, and they look closely at who performed the exam. When your school’s compliance team reviews your file, a physician’s signature answers that question cleanly: a doctor examined you, and a doctor signed your form.
At Monarch Medicine, a physician on our team — led by Dr. Lisa Clay, MD, board-certified in Family Medicine — examines every healthcare student and signs the form. We complete what we can during your visit; if anything is lab-pending, it follows through MyChart.
“Healthcare students are about to spend their careers caring for patients. The least we can do is make their entry into the profession smooth, dignified, and properly documented.” — Dr. Lisa Clay, MD

WHAT YOUR PROGRAM REQUIRES — WHAT WE PROVIDE
Healthcare Student Physicals & Documentation
Starting nursing school, medical assistant training, or another healthcare program? Bring your program’s forms, immunization records, and testing requirements to Monarch Medicine. We provide physician-performed physicals, TB testing, and blood draws for immunity titers to help you complete your requirements.
Healthcare Student Physical
A Monarch physician performs your physical and reviews your program’s paperwork. The self-pay physical is $153.30. Testing, vaccines, and other services are additional. Bring every page of your school’s required forms.
Immunity Titers & Blood Draws
We draw blood for immunity titers and other ordered tests sent to LabCorp. Bring your program’s list so the physician can review which tests you need. Blood collection is $8.40, and LabCorp bills its tests separately. Results return after your visit.
TB Testing — By Appointment
We offer TB skin testing and blood draws for QuantiFERON testing sent to LabCorp. Call to schedule and tell us which test your program requires. Skin testing requires a return visit for the reading; two-step testing requires additional visits. Bring any previous TB results.
Immunization Records & Tdap
Bring your immunization history so we can review it against your program’s requirements. We offer Tdap vaccination when appropriate and available. Arrange other required vaccines, including flu shots, through a pharmacy or your primary care provider.
Plan Ahead for Your Deadline
We complete the portions of your paperwork supported by your exam and available records. Final documentation may need to wait for lab results, TB readings, missing records, or additional evaluation.
Call (317) 804-4203 with your requirements and deadline so we can help you plan your visit.
TB TESTING
TB Testing & Evaluation for Your Program
Bring your school’s requirements and prior TB results. Our physician will review the appropriate test or evaluation. Call to schedule TB testing and required readings.
All TB testing is self-pay only and is not billed to insurance.
PPD Skin Testing
$100 one-step · $200 two-step
One-step testing includes one placement and one reading for $100. A full two-step series includes two placements and two readings for $200 total. Each reading is needed 48–72 hours after placement. When a second step is needed, it is placed 1–3 weeks after the first test is read.
Plan ahead: call to arrange placements and readings around your program’s deadline.
QuantiFERON-TB Gold
$400 total — self-pay
Includes blood collection and laboratory testing. We collect your sample and send it to LabCorp. Monarch handles payment to LabCorp, so you will not receive a separate LabCorp bill for this test. No return visit is needed for a skin-test reading. Results return after your visit; turnaround varies.
A TB blood test is preferred for people with BCG vaccination history. Bring any prior positive TB results for physician review before repeat testing.
Chest X-Ray & Physician Evaluation
$310.80 self-pay
Includes an office visit and on-site chest X-ray when the physician determines imaging is needed. Bring prior positive TB results and any previous chest X-ray reports. Imaging is part of an evaluation when indicated; it does not replace a TB blood or skin test for routine infection screening.
BCG vaccination alone does not mean you need a chest X-ray. Final documentation depends on the evaluation and available results.
