A Monarch physician performs the pre-op evaluation based on your health history, physical exam, current medications, surgical form, and any tests requested by your surgeon. We complete the documentation and send it back so your surgical and anesthesia team can review it before your procedure.

WHEN MONARCH IS NOT THE RIGHT PLACE

We Tell You What We Don't Do

Pre-op evaluation at Monarch is appropriate for many outpatient and routine surgeries. Some situations need cardiology, anesthesia-led evaluation, or ER-level care. Honest framing first:

Cardiac clearance → If your surgeon specifically requires cardiology clearance, stress testing, echocardiogram interpretation, or management of complex heart disease, you may need a cardiologist.

High-risk surgery or unstable symptoms → Chest pain, severe shortness of breath, fainting, new neurologic symptoms, uncontrolled blood pressure, or symptoms suggesting an emergency should be evaluated in the ER or by the appropriate specialist.

Complex anesthesia concerns → Prior anesthesia complications, severe sleep apnea, complex airway history, significant heart or lung disease, or major inpatient surgery may require anesthesia-directed evaluation through your surgeon's system.

Missing surgeon requirements → Bring the surgeon's form. We need to know what exam, labs, EKG, or documentation the surgeon requires before we can complete the paperwork. Without the form, we cannot guarantee the evaluation will meet your surgeon's requirements.

A pre-op visit is not paperwork. It is a checkpoint. Your surgeon needs to know your medical history, your medications, your heart and lung status, and how you have done with anesthesia before. The exam, the labs, and the EKG are how we make sure your surgical and anesthesia team has what they need.

A Monarch physician performs the evaluation, completes the documentation, and sends it back to your surgeon. We do not clear you for surgery — that decision belongs to your surgeon and anesthesia team. We make sure they have what they need to make it.

What Happens If Findings Are Noted

If Dr. Clay identifies a medical condition that may be relevant to your procedure — such as elevated blood pressure, an EKG finding, or an unmanaged condition — this will be thoroughly documented in your evaluation. Your surgical team will be notified. Depending on the finding, specialist follow-up may be recommended before your surgery proceeds. The evaluation fee applies regardless of outcome. Our goal is always to help you reach surgery safely and with complete information.

Do I need an appointment?

Yes. Pre-op visits are appointment-based, not walk-in. Call (317) 804-4203 with your surgery date, surgeon's name, procedure, and your surgeon's requirements so we can plan the visit.

What should I bring?

Bring your surgeon's pre-op form, a current list of your medications, your allergies, prior surgeries, and any test results your surgeon's office has already shared with you. The form is what tells us what your surgeon requires.

Does Monarch “clear” me for surgery?

A Monarch physician performs your pre-op evaluation, completes the documentation, and sends it to your surgeon. Your surgeon and anesthesia team make the final decision about proceeding with surgery based on the evaluation, your medical history, and the procedure planned. Some surgeons call this workflow “surgical clearance” — the process is the same.

Can you do an EKG and blood draw at the visit?

Yes, when your surgeon requires them. EKG is performed in our office. Blood draw is collected at our office and sent to LabCorp; LabCorp bills the lab cost separately, and results often return in 1 to 3 days but timing can vary.

How does the surgeon get the results?

Your H&P, the completed surgeon's form, and the EKG (if performed) are sent back to your surgeon's office by fax or secure document delivery. Lab results follow when LabCorp returns them.

What if you find something concerning during the evaluation?

If the evaluation suggests cardiology, pulmonology, or anesthesia-directed review is needed, a Monarch physician will discuss it with you, document it, and notify your surgeon so they can decide next steps before surgery. We do not delay your surgery decision — that is your surgeon's call — but we make sure they have the right information.

Does Monarch do cardiology clearance?

No. If your surgeon specifically requires cardiology clearance, stress testing, echocardiogram interpretation, or management of complex heart disease, you may need a cardiologist. We can perform the standard pre-op physical and EKG, but cardiac clearance is a different scope of evaluation.

Do you accept insurance?
