What Is a Pre-Op Evaluation?
A pre-operative evaluation is a physician-led medical assessment performed before a planned surgical procedure. Your surgeon or surgical center requires this to understand your current health status and identify any factors that may affect your procedure. At Monarch Medicine, every evaluation is performed by a board-certified physician who documents findings and communicates directly with your surgical team.
History & Physical Exam
Dr. Clay reviews your complete medical history, current medications, allergies, and performs a thorough physical examination tailored to your surgeon's pre-op form requirements. Completed same visit.
Surgical Risk Assessment
Dr. Clay identifies and documents any medical conditions that may be relevant to your procedure or anesthesia. Findings are communicated to your surgical team to support their planning.
Results & Communication*
Your evaluation results are faxed directly to your surgeon's office. You receive access via MyChart. If labs are ordered, results typically return within 1–3 business days.
*Physical exam and EKG (if ordered) completed same visit. If lab work is ordered, results return within 1–3 business days before final documentation is issued to your surgical team. X-ray results up to 4 days.

Dr. Clay performs a complete history and physical, reviews your medications and conditions, and completes your surgeon's pre-op form. EKG performed same visit if ordered.

Testing If Ordered
Blood draw completed same visit — LabCorp processes results in 1–3 business days. X-ray results up to 4 days. You may use your own lab provider.

Results Sent to Surgeon
Once all results are available, we fax documentation directly to your surgeon's office. You receive access to your results through MyChart. Bring your surgeon's fax number to your appointment.

What Happens If Findings Are Noted
If Dr. Clay identifies a medical condition that may be relevant to your procedure — such as elevated blood pressure, an EKG finding, or an unmanaged condition — this will be thoroughly documented in your evaluation. Your surgical team will be notified. Depending on the finding, specialist follow-up may be recommended before your surgery proceeds. The evaluation fee applies regardless of outcome. Our goal is always to help you reach surgery safely and with complete information.

Pre-Op Evaluation FAQ
How do I schedule a pre-op evaluation?
Call (317) 804-4203 to schedule your appointment. Appointments are required — we do not accept walk-ins for pre-op evaluations. Our team will confirm availability and tell you what to bring.
How far in advance should I schedule?
Most surgeons require your evaluation results before surgery. If labs are ordered, allow 1–3 business days for results. If X-ray is ordered, allow up to 4 days. We recommend scheduling at least 1–2 weeks before your procedure to allow adequate time for all results to return to your surgical team.
What does the evaluation include?
Your evaluation includes a complete history and physical exam, surgical risk assessment, medication review, and completion of your surgeon's pre-op form. EKG, blood work, or X-ray are ordered based on your surgeon's requirements and clinical findings and are billed in addition to the base fee.
Will I leave with my results?
Your physical exam is completed same visit. Results — including lab work and imaging if ordered — are sent directly to your surgeon's office by fax once all results are available. You receive access to your results through MyChart. Please bring your surgeon's fax number to your appointment.
Do you accept insurance for pre-op evaluations?
Pre-op evaluations at Monarch Medicine are self-pay only at $215.60 base fee. Payment is due at the time of your visit — cash, credit, or debit. Lab work processed through LabCorp is billed separately by LabCorp and may be submitted to your insurance independently.
What if findings are noted during my evaluation?
If Dr. Clay identifies a condition that may be relevant to your procedure, it will be documented and communicated to your surgical team. Specialist follow-up may be recommended before surgery proceeds. The evaluation fee applies regardless of outcome. Our goal is to help you reach surgery safely and with complete information.
Can I travel from outside Carmel for my evaluation?
