What’s Included in Your Sports Physical
Every sports physical at Monarch Medicine is a physician-performed exam — not a quick checkbox review. A Monarch physician evaluates your child’s fitness for athletic competition and screens for conditions that could put them at risk on the field.
Cardiovascular Screening
Heart rate, blood pressure, heart and lung auscultation, screening for murmurs or arrhythmias. This is the most critical component — sudden cardiac events are the leading cause of death in young athletes.
Musculoskeletal Assessment
Joint flexibility, range of motion, prior injury evaluation, and screening for conditions like scoliosis or ligament laxity that could increase injury risk during competition.
Medical History & Risk Review
Family history of cardiac events, concussion history, asthma, heat-related illness risk, current medications, and any conditions requiring clearance modifications or sport-specific restrictions.

Why a Physician-Led Sports Physical Matters
A sports physical has one job: make sure your athlete is safe to compete. That takes more than a signature. It takes listening to the history, examining carefully, and knowing what a finding means for a specific sport — a subtle heart murmur, a lingering concussion symptom, a joint that never fully recovered from last season.
At Monarch Medicine, sports physicals are performed by a physician. Dr. Lisa Clay, MD is a board-certified family physician with nearly two decades of clinical experience caring for patients of all ages, including young athletes. If the exam raises a question, we have on-site digital X-ray and can order labs or refer to a specialist when indicated — and we explain what we find in plain language before you leave.

Complete cardiovascular, musculoskeletal, and neurological screening performed by Monarch Medicine Physician. On-site X-ray available if a concern is identified.

When the exam supports clearance — and most do — forms are completed and signed before you leave. If something needs a closer look, the physician explains the next step and what it means for the season.

Sports Physical FAQ
Do I need an appointment for a sports physical?
No. Walk in during business hours (Mon–Fri 8am–6pm, Sat–Sun 9am–12pm). Most patients are seen within 15 minutes. You can also check in online through MyChart to hold your spot.
How long does the exam take?
The physician exam takes approximately 15–20 minutes. Total visit time including check-in and form completion is typically 30 minutes or less.
Does my child need a sports physical every year?
Yes. Under IHSAA Rule 3-10, Indiana high school athletes need a new physical each school year, completed on or after April 1 and before the first practice. Most middle school and club programs require an annual physical as well. A visit after April 1 covers fall, winter, and spring seasons for the upcoming school year with one exam. Can you complete my school’s specific form?
Yes. Bring your school, league, or club’s physical form and the physician will complete and sign it during your visit. We handle IHSAA forms, AAU forms, club sports forms, college athletic forms, and other pre-participation physical forms — and we print the current IHSAA form on-site if you don’t have one.
What if something is found during the exam?
The physician will discuss it with you right away — whether that’s an abnormal heart sound, elevated blood pressure, or an unresolved prior injury. We have on-site X-ray and can order labs or refer to a specialist when indicated. Most athletes are cleared the same visit, but we won’t sign a clearance if further evaluation is needed for your child’s safety.
Do you do sports physicals for adults?
Yes. We provide pre-participation exams for adult recreational leagues, fitness clearance, return-to-sport evaluations after injury, and other activity clearance needs.
Is this covered by insurance?
Sports physicals at Monarch are a flat-rate self-pay service: $65.80, paid at your visit. We don’t bill insurance for sports physicals, which is what keeps the price simple. Urgent care visits are different — see the insurance plans we accept →
