PPD Skin Test (Mantoux)
The Standard Tuberculin Skin Test
How it works: A small amount of tuberculin purified protein derivative (PPD) is injected just under the skin of the forearm. You return in 48–72 hours so a licensed clinician can measure and interpret the reaction.
Turnaround: 48–72 hours (requires a return visit for reading)
Accepted for: School or employer screening when the form permits a skin test
1-Step & 2-Step available: We perform both single-step and two-step Mantoux tests as required by your employer or facility

QuantiFERON Gold (IGRA Blood Test)
Single-Visit Blood Draw — No Return Trip
How it works: A standard blood draw is collected and sent to a reference lab. The interferon-gamma release assay (IGRA) measures your immune response to TB-specific proteins — no skin injection required.
Turnaround: 2–3 business days
Preferred for: Patients with prior BCG vaccination, those who cannot return for a 48–72 hour reading, healthcare workers requiring IGRA per facility policy
CDC recommended: The CDC considers IGRA the preferred test for most healthcare personnel screening
Only one visit required — ideal for busy employers
Chest X-Ray (Positive Result Follow-Up)
Required After Any Positive TB Test
How it works: If your PPD or QuantiFERON result is positive, Indiana regulations require a chest X-ray to rule out active pulmonary tuberculosis. A Monarch physician reviews the imaging and provides the evaluation and clearance letter.
Turnaround: Same-day imaging with physician review
Includes: Chest radiograph, physician review, written clearance letter for your employer
No referral needed: If your test is positive, we schedule the chest X-ray here and handle the complete follow-up workflow in one location

TB Testing Pricing
All TB testing is self-pay only and is not billed to insurance. Call with your school or employer’s requirements so we can coordinate your appointment and required readings. QuantiFERON is $400 total, including blood collection and laboratory testing; you will not receive a separate LabCorp bill for this test.
PPD Skin Test
$100 per Test
One-step testing is $100, including one placement and reading. Two-step testing is $200 total, including both placements and readings. Tell us which your program requires so we can coordinate your visits.
QuantiFERON Blood Test
$400 Total — Self-Pay
Includes blood collection and laboratory testing. We collect your sample and send it to LabCorp. Monarch handles payment to LabCorp, so you will not receive a separate LabCorp bill for this test. Results return after laboratory processing; timing varies.
Chest X-Ray & Evaluation
$310.80*
A simple new-patient visit at $153.30 plus imaging at the published $157.50 X-ray rate. Your physician determines the evaluation and follow-up needed.

Why Physician Oversight Matters for TB Testing
TB testing seems simple, but interpretation errors are common — and the consequences for employers and patients are serious. A false positive can cost an employee their job start date. A missed positive can put an entire facility at risk.
A Monarch physician reviews positive results and determines the necessary follow-up. On-site X-ray is available when indicated, and documentation reflects the completed evaluation. Some patients need additional testing or specialist/public health care.

TB Testing FAQ
Do I need an appointment for a TB test?
Yes. Call (317) 804-4203 to schedule. Our team coordinates your test and any return visits needed for readings.
What should I bring?
Bring your school or employer’s current form, photo ID, and previous TB results. If you have had a positive test, bring the original report and any related chest-imaging or treatment records.
What is the difference between a PPD skin test and QuantiFERON?
A PPD skin test is placed under the skin of your forearm and must be read by trained staff 48–72 hours later. QuantiFERON is a blood test collected at Monarch and sent to LabCorp. It does not require a skin-test reading visit, but results return after laboratory processing.
Which test does my school or employer require?
Requirements vary by program and workplace. Bring your current instructions so we can review the requested test, your history, and the documentation needed. Your organization determines which documentation it accepts.
Do you offer two-step TB skin testing?
Yes. When two-step testing is required, the second test is placed 1–3 weeks after the first negative result is read. Each placement needs a reading 48–72 hours later, usually requiring four visits altogether. Call to coordinate the visits and confirm the total price.
I had a BCG vaccine. Which test should I ask about?
TB blood testing is preferred for people who have received BCG. Tell your physician about your vaccination history and bring your program’s requirements.
What if I already had a positive TB test?
Bring your previous result and any chest-imaging or treatment records. A documented prior positive result generally does not require another skin or blood test. Your physician reviews your history, symptoms, and any follow-up needed.
Does a positive screening result mean I have active TB?
No. A positive screening result requires further evaluation. Your physician determines whether you need additional testing, chest imaging, or referral before documentation can be completed.
Can you complete my employer’s form?
We document completed testing and the physician’s findings. Final paperwork may need to wait for readings, laboratory results, or additional evaluation. Bring every page of your current form.
Can I schedule a weekend appointment?
Call to confirm weekend availability. Monarch is open Saturday and Sunday from 9am to 12pm. Our team coordinates any required reading visit when scheduling your test.
How much does TB testing cost?
All TB testing is self-pay only and is not billed to insurance. One PPD placement and reading costs $100. Two-step PPD testing costs $200 total, including both placements and readings. QuantiFERON-TB Gold costs $400 total, including blood collection and laboratory testing, with no separate LabCorp bill. Any additional evaluation or imaging is priced separately.
