Medicaid & Humana

Monarch Medicine is not in network with Medicaid or Humana plans. We do not bill these plans, and we cannot accept them as payment for visits.

If you have one of these plans, you have two options:

  1. Self-pay at the time of visit. Our self-pay prices are published, with no facility fees and clear in-office pricing before treatment.
  2. Use a clinic that accepts your plan. CVS MinuteClinic and Indy Now have locations that accept Medicaid and Humana for some services.

Medicare Patients

Monarch is an opt-out provider for Medicare. We do not bill Medicare directly. Medicare patients can be seen as self-pay — you sign a one-time Medicare opt-out contract at your first visit, and our self-pay rates apply.

Most urgent care chains are built around insurance billing first and patient care second. Visits get coded for maximum reimbursement, prices stay hidden until the bill arrives, and patients without coverage get treated as second class.

Monarch was built the other way around. Insurance is a tool for patients who have it. Self-pay is a real, dignified option for patients who don't. Both paths get you the same physician, the same on-site X-ray, the same time with the doctor, the same care.

HOW BILLING WORKS

Three Paths, One Standard of Care

No matter how you pay, the visit is the same.

If You Have Insurance

  1. Bring your insurance card to your visit.
  2. We verify your coverage and copay before you see the physician.
  3. You pay your copay at check-out.
  4. We bill your insurance for the rest.
  5. Any deductible or coinsurance you owe arrives in a statement after your insurance processes the claim.

If You Are Self-Pay

  1. You see published prices on our self-pay page before your visit.
  2. Final cost is confirmed at check-in.
  3. Payment is collected at time of service.
  4. No bill arriving weeks later. No facility fees.
  5. If your visit ends up more complex than expected, we tell you before adding any service.

If You Use HSA or FSA

  1. Bring your HSA or FSA debit card.
  2. All medical services on this site qualify for HSA and FSA payment.
  3. You can pay your insurance copay or your full self-pay rate using HSA / FSA.
  4. We provide an itemized receipt for HSA / FSA reimbursement if needed.

Wellness programs (VIB, NAD+, GLP-1) may have HSA/FSA eligibility limits — check with your plan.

Insurance & Payment FAQs

How do I know if my plan is accepted?

Call (317) 804-4203 with your insurance card before your visit and we will verify in-network status. Most Anthem BCBS, UnitedHealthcare, Aetna, Cigna, and Encore/IU Health Plans are accepted.

What is my copay going to be?

Copays vary by plan. We verify your copay at check-in before you see the physician, so you know exactly what you owe at time of service. Deductibles and coinsurance, if applicable, arrive in a statement after your insurance processes the claim.

Can I pay self-pay even if I have insurance?

Yes. Some patients prefer self-pay for privacy reasons (such as STD testing) or because their high deductible makes self-pay cheaper than what they would owe through insurance. We can run the math both ways at check-in.

Do you accept HSA and FSA cards?

Yes. HSA and FSA cards are accepted for all medical services. You can use them to pay copays, deductibles, or full self-pay rates. Wellness programs (VIB, NAD+, GLP-1) may have plan-specific eligibility limits.

What if my insurance denies the claim?

If your insurance denies coverage for a service that was covered in good faith, we work with you on payment options. Our self-pay rates serve as the cap — you will never owe more than the self-pay price for a denied claim.

Why are you not in network with Medicaid or Humana?

Insurance contracts are negotiated. Medicaid and Humana reimbursement rates do not support physician-led, walk-in urgent care at our cost structure. Patients with these plans can still be seen at Monarch as self-pay patients at our published rates.

Do you offer payment plans?

For most visits, payment is collected at time of service. For larger services like pre-op evaluations or wellness programs, talk to our front desk — we work with patients on a case-by-case basis.
