Strep Throat vs. Viral Sore Throat — How to Tell the Difference
Not every sore throat is strep. A Monarch physician performs a rapid strep test and clinical evaluation to determine whether antibiotics are needed — because treating a viral sore throat with antibiotics doesn’t help and contributes to antibiotic resistance.
Strep Throat (Bacterial)
- Sudden onset of severe sore throat
- Pain when swallowing
- Fever (101°F or higher)
- Red, swollen tonsils with white patches or streaks
- Swollen lymph nodes in the neck
- Headache, body aches, sometimes nausea
- Usually NO cough, runny nose, or congestion
Treatment: Antibiotics (penicillin or amoxicillin is first-line). Symptoms improve within 24–48 hours of starting antibiotics.
Viral Sore Throat
- Gradual onset
- Sore throat with cough, runny nose, congestion
- Hoarse voice or laryngitis
- Low-grade or no fever
- Sneezing, watery eyes
- Usually resolves in 5–7 days
Treatment: Rest, fluids, OTC pain relief. Antibiotics are NOT effective for viral infections.

Why a Rapid Strep Test Alone Isn’t Enough
A rapid strep test helps identify Group A Streptococcus, but your symptoms and examination also matter. A negative result may need follow-up testing, and other causes of a severe sore throat may require a different evaluation.
At Monarch Medicine, a Monarch physician examines your throat and neck, reviews your symptom timeline, and assesses findings such as white patches on the tonsils or swollen lymph nodes. The physician also checks for signs of complications and determines the appropriate treatment.

WHEN TO VISIT
When to Get Tested for Strep
Not every sore throat needs a strep test. But visit Monarch Medicine if you or your child has:
- ✓ Sudden severe sore throat without cough or congestion
- ✓ Fever of 101°F or higher with sore throat
- ✓ Painful swallowing or difficulty swallowing
- ✓ Swollen or tender lymph nodes in the neck
- ✓ White patches or red spots on the tonsils or back of throat
- ✓ Known exposure to someone with confirmed strep
- ✓ A child with sore throat, stomachache, and headache (classic pediatric presentation)
- ✓ Sore throat lasting more than 48 hours without improvement
Important: Untreated strep throat can lead to serious complications including rheumatic fever and kidney inflammation. If you suspect strep, get tested — don’t wait it out.

Strep Throat FAQ
How long does a rapid strep test take?
About 5 minutes after the throat swab. You receive the rapid result during your visit. This is the test processing time; total visit time also includes check-in, the physician evaluation, and your treatment plan.
Can strep throat go away without antibiotics?
Confirmed strep throat should be treated with antibiotics. Treatment shortens symptoms, reduces transmission, and lowers the risk of complications such as rheumatic fever. Antibiotics do not treat viral sore throats.
How soon after starting antibiotics will I feel better?
Many patients start feeling better within 24–48 hours. Take your medication exactly as prescribed and contact the clinic if symptoms worsen or are not improving.
Can adults get strep throat?
Yes. Strep throat is most common in school-age children, but adults can get it too, especially through close contact with children. Monarch evaluates sore throats in both adults and children.
What if my rapid strep test is negative but I still feel terrible?
A negative rapid antigen test can miss strep. Symptomatic children age 3 and older should have a backup throat culture. Adults do not routinely need one. Your physician determines the next step and evaluates other possible causes of your symptoms.
Is strep throat contagious?
Yes. Stay home until you are fever-free and have taken appropriate antibiotics for at least 12–24 hours. Follow your physician’s instructions and your school or workplace’s requirements.
Do you accept insurance for strep throat visits?
Yes. We accept most major insurance plans including Anthem, Aetna, UnitedHealthcare, and Cigna. We are not in network with Medicaid or Humana. We also offer affordable self-pay pricing.
