Strep Throat: Symptoms, Rapid Testing & Same-Day Treatment | Monarch Medicine Carmel
Strep throat is one of the most common reasons patients walk into Monarch Medicine — and one of the most important to diagnose correctly. Untreated strep can lead to serious complications including rheumatic fever and kidney inflammation. The good news: a rapid strep test takes minutes, and if positive, you leave with antibiotics the same day.
I’m Dr. Lisa Clay, board-certified family physician and founder of Monarch Medicine Urgent Care in Carmel. Here’s what I want every patient to know about strep throat.
Strep Throat Symptoms
Strep throat (Group A Streptococcus) presents differently from a viral sore throat. Here’s how to tell them apart:

The key clinical clue: If your sore throat came on suddenly with fever and WITHOUT a cough or runny nose, strep is highly likely. Cough and congestion point toward a viral cause.
How Strep Throat Is Diagnosed
You cannot diagnose strep by looking at the throat alone — a test is required. At Monarch Medicine, we use two methods:
Rapid Antigen Detection Test (RADT)
A quick throat swab with results in 5–10 minutes. If positive, we start antibiotics immediately. Sensitivity is approximately 95% in our hands — meaning a positive result is highly reliable.
Throat Culture (Backup)
If the rapid test is negative but clinical suspicion is high (classic symptoms, known strep exposure), we can send a throat culture for confirmation. Results take 24–48 hours.
Why testing matters: Antibiotics only work against bacterial strep — not viral sore throats. Prescribing antibiotics without testing contributes to antibiotic resistance and exposes you to unnecessary side effects. A responsible physician tests before prescribing.
Treatment: What Happens After a Positive Test
If your rapid strep test is positive, your treatment plan typically includes:
- Antibiotics — typically amoxicillin or penicillin for 10 days. Prescription sent to your pharmacy before you leave the visit.
- Pain management — over-the-counter ibuprofen or acetaminophen for fever and throat pain
- Hydration guidance — warm liquids, soft foods, popsicles for children
- Return-to-school/work guidance — typically 24 hours after starting antibiotics and fever-free. We provide clearance notes same-day.
Most patients feel significantly better within 48 hours of starting antibiotics. The full 10-day course is essential even if you feel better — stopping early increases the risk of complications and recurrence.
Strep in Children
Strep throat is most common in children ages 5–15, and it often presents differently than in adults. Children may have nausea, vomiting, or abdominal pain as their primary symptoms — without a severe sore throat complaint. A child who says “my tummy hurts” and has a fever may actually have strep.
We see children of all ages at Monarch Medicine — no age restrictions, no referrals needed. If your child’s school reports a strep outbreak, walk in for a rapid test even if symptoms are mild.
When Strep Keeps Coming Back
Recurrent strep throat (3+ episodes in a year) is a pattern we evaluate carefully. Possible causes include incomplete antibiotic courses, household carriers (a family member who carries strep but isn’t symptomatic), or tonsils that harbor bacteria. Your physician will discuss whether ENT referral for tonsillectomy evaluation is appropriate.
Complications of Untreated Strep
Left untreated, strep throat can lead to:
- Peritonsillar abscess — a painful, pus-filled pocket behind the tonsil that may require drainage
- Rheumatic fever — inflammatory condition affecting the heart, joints, and nervous system (rare but serious)
- Post-streptococcal glomerulonephritis — kidney inflammation
- Scarlet fever — strep infection with a characteristic sandpaper-like rash
This is why testing and treatment matter — even when a sore throat seems minor.
