The ER is designed for life-threatening emergencies—and for non-emergency pediatric conditions, it means long waits, high costs, and an environment that isn’t designed for children. Fever in Children: When to Come In

Fever is the most common reason parents bring children to Monarch Medicine after hours. Here’s the clinical guidance Dr. Clay uses for fever thresholds:

  • Infant under 3 months with any fever (100.4°F / 38°C or higher): Go to the ER immediately. Neonatal fever requires sepsis workup that urgent care is not equipped to perform.
  • Infant 3–6 months with fever above 102°F: Come in same day for evaluation.
  • Child over 6 months with fever above 102°F lasting more than 2 days, or any fever accompanied by rash, difficulty breathing, stiff neck, significant lethargy, or severe ear/throat pain: Come in for evaluation.
  • Fever above 104°F in any child over 3 months: Come in same day even if the child otherwise appears well.
  • Fever that responds to ibuprofen/acetaminophen and the child seems comfortable: Monitor at home. Return if fever persists beyond 3 days or symptoms worsen.
