Sports Physicals Every Day at Monarch Medicine Urgent Care

Student athletes in Carmel, Fishers, Westfield, and Zionsville need sports physicals every year before the season starts — and most families don’t have time to schedule a separate appointment weeks in advance. Monarch Medicine offers sports physicals every day, walk-in, no appointment needed, for $65.80. Your athlete is evaluated by Dr. Lisa Clay, MD, FAAFP — a board-certified family physician — and all required clearance forms are completed on-site before you leave.

I’m Dr. Lisa Clay, and I’ve made sports physicals available every day at Monarch Medicine because tryout deadlines and season start dates don’t always align with a pediatrician’s availability. No family should have to choose between missing a season and paying ER prices for a routine physical. $65.80, walk in, done.

What’s Included in a Sports Physical at Monarch Medicine

Every sports physical at Monarch Medicine is a complete Pre-Participation Physical Evaluation (PPE) that meets IHSAA clearance requirements and Hamilton County school standards:

Component What We Assess
Medical History Review Prior injuries, surgeries, chronic conditions (asthma, diabetes, heart conditions), current medications, family cardiac history
Vital Signs Height, weight, BMI, blood pressure, pulse, oxygen saturation
Cardiovascular Screening Heart rate and rhythm assessment, blood pressure evaluation, review of cardiac symptoms and family history of sudden cardiac death
Musculoskeletal Exam Joint range of motion, prior injury assessment, strength and stability evaluation for sport-specific demands
Vision Screening Visual acuity check; referral recommended if significant deficit identified
Respiratory Assessment Lung auscultation, review of asthma or exercise-induced bronchospasm history, inhaler status
Clearance Documentation IHSAA Pre-Participation Physical Evaluation form completed and signed on-site; school-specific forms accepted

Most visits take 20 minutes or less. Walk in, be seen by a physician, leave with your clearance form. If Dr. Clay identifies a finding that warrants further evaluation before clearance — a heart murmur, an unresolved prior injury, elevated blood pressure — we explain clearly what the next step is and why, so nothing is left ambiguous.

Why Cardiac Screening Matters in a Sports Physical

Sudden cardiac arrest is the leading cause of death in student athletes during exercise — and the majority of cases involve conditions that were present but undetected before the event. The cardiovascular component of the Pre-Participation Physical Evaluation is specifically designed to screen for the conditions most associated with exercise-related sudden cardiac death: hypertrophic cardiomyopathy, long QT syndrome, anomalous coronary arteries, and Wolff-Parkinson-White syndrome.

According to the American Heart Association, a 14-point cardiac history and physical examination is the recommended minimum screening standard for student athletes. Monarch Medicine’s sports physical includes this assessment at every visit. Athletes with symptoms — chest pain during exercise, unexplained fainting, palpitations, or a family history of sudden cardiac death under age 50 — receive additional evaluation and cardiology referral if indicated.

Update Immunizations at the Same Visit

If your athlete is behind on required school vaccinations — Tdap, meningococcal, HPV — we can update them at the same sports physical visit through our on-site vaccination services. No second trip, no separate pharmacy visit. Bring your child’s immunization record and we’ll identify any gaps at the time of the physical.

Sports We Clear — All Student Athletes Welcome

Monarch Medicine completes Pre-Participation Physical Evaluations for all IHSAA-regulated sports and activities, school-specific physical forms, and camp and activity physicals. We serve student athletes from Carmel High School, Hamilton Southeastern, Westfield High School, Zionsville Community High School, and schools throughout Hamilton County. If you’re unsure whether we complete your specific form, call us at (317) 804-4203 before coming in.

We also complete physicals for adult recreational leagues, employer fitness requirements, and camp programs — not just school-age athletes. Same walk-in process, same same-day documentation.

What to Bring to Your Sports Physical

  • Your school’s specific physical form if different from the standard IHSAA PPE form — we complete what you bring
  • Your child’s immunization record — so we can identify any vaccination gaps at the same visit
  • Insurance card or payment — $65.80 cash, card, or insurance
  • A list of current medications and any known medical conditions or prior injuries
  • Parent or guardian present for athletes under 18
