Viral vs. Bacterial Sinusitis — Why It Matters
Most sinus infections start as viral and resolve on their own. Antibiotics only work for bacterial sinusitis — and prescribing them unnecessarily contributes to antibiotic resistance. Dr. Clay determines which type you have before recommending treatment.
Viral Sinusitis
- Follows a cold or upper respiratory infection
- Symptoms typically improve within 7–10 days
- Clear or white nasal discharge
- Mild to moderate facial pressure
- Low-grade or no fever
Treatment: Nasal saline irrigation, decongestants, rest, symptom management. Antibiotics are NOT indicated.
Bacterial Sinusitis
- Symptoms lasting 10+ days without improvement
- Symptoms that worsen after initial improvement (“double worsening”)
- Thick, yellow-green nasal discharge
- Severe facial pain or pressure, especially one-sided
- Fever above 102°F with purulent discharge
Treatment: Targeted antibiotics (amoxicillin-clavulanate is first-line), nasal steroids, symptom management.

How do I know if my sinus infection is bacterial or viral?
Viral sinusitis typically improves within 7–10 days. Bacterial sinusitis is suspected when symptoms last more than 10 days, worsen after initial improvement, or include high fever with thick discolored discharge. Dr. Clay performs a clinical evaluation to determine the type and recommend the right treatment.
Do I need antibiotics for a sinus infection?
Most sinus infections are viral and do not require antibiotics. Antibiotics are only effective for bacterial sinusitis. Dr. Clay will determine whether antibiotics are appropriate based on your symptoms, timeline, and physical examination.
Can urgent care treat a sinus infection?
Yes. Sinus infections are one of the most common conditions we treat at Monarch Medicine. Dr. Clay provides physician-level evaluation, prescriptions when needed, and on-site X-ray if complications are suspected.
What can I do at home to relieve sinus pressure?
Nasal saline irrigation (neti pot or squeeze bottle), warm compresses over the sinuses, steam inhalation, staying hydrated, and OTC decongestants can help relieve pressure. If symptoms persist beyond 10 days or worsen, come in for evaluation.
How long does a sinus infection last?
Viral sinusitis typically resolves in 7–10 days. Bacterial sinusitis may last 10–14 days or longer without treatment. With appropriate antibiotics for bacterial cases, most patients feel significantly better within 48–72 hours.
I keep getting sinus infections. What should I do?
Recurrent sinusitis (3+ episodes per year) may indicate an underlying issue such as allergies, nasal polyps, or anatomical factors. Dr. Clay can evaluate your history, provide acute treatment, and refer to an ENT specialist if further workup is warranted.
