WHY THE SIGNATURE MATTERS
Your School Form Gets Signed By a Physician.
Most clinical placement coordinators have seen physical forms signed by physician assistants and nurse practitioners. They’ll accept them. But when documentation gets reviewed and your school’s liability team asks who actually examined you, an MD signature carries different weight than an NP signature.
At Monarch Medicine, Dr. Lisa Clay, MD — board-certified Family Medicine and Fellow of the American Academy of Family Physicians — or another physician on our team examines every healthcare student personally. The signature on your form is from a doctor who actually saw you.
“Healthcare students are about to spend their careers caring for patients. The least we can do is make their entry into the profession smooth, dignified, and properly documented. Bring me your form, I’ll sign it that day.” — Dr. Lisa Clay, MD
WHAT YOUR PROGRAM REQUIRES — WHAT WE PROVIDE
Healthcare Student Documentation, Done in One Visit
Most nursing programs, MA training programs, and allied health programs require some combination of these. We provide all of them in-office. For titers and vaccines, we’ll point you to a nearby pharmacy.
Healthcare Student Physical
Complete physical exam, vitals, vision, and clearance for clinical work. Documentation completed during your visit.
$105.70 self-pay
TB Testing — Three Options
Two-step PPD, Quantiferon-TB Gold blood test, or chest X-ray for TB exclusion. Match the test to your school’s requirement.
$100 – $400 self-pay
Tdap Booster
Required by most programs if it has been more than ten years since your last Tdap. Administered on-site, documented same visit.
$88.46 self-pay
Titers & Vaccines — We’ll Refer You to a Pharmacy
We don’t draw titer blood or stock MMR, Varicella, or Hep B vaccines in office. Most pharmacies around Carmel and Indianapolis — CVS, Walgreens, Kroger, Meijer — can complete titers and administer vaccines faster and at lower cost than we could. Bring your school’s requirement form to the pharmacy and they’ll document everything in a format your school will accept.
TB TESTING
Three TB Tests — Pick the One That Fits Your Schedule
Different schools require different TB tests. We offer all three. Bring your form — we’ll match the test to the requirement.
Two-Step PPD Skin Test
$100 self-pay
Most affordable. Two visits: placement, then a 48–72 hour read. The read takes 5 minutes and doesn’t require an appointment — walk in. Read fee is included. Most common school requirement.
Best for: students with a flexible schedule who can return in the 48–72 hour window.
Quantiferon-TB Gold
$400 self-pay
One visit, no return trip. Blood draw, results in 24–48 hours. The right choice if your schedule won’t allow a 48-hour return, you’ve had a positive PPD before, or your school specifically requires Quantiferon (also called QFT or interferon-gamma release assay).
Best for: students with packed clinical schedules or BCG vaccination history.
Chest X-Ray TB Exclusion
$157.50 self-pay
We have on-site digital X-ray. Often the right choice for students with prior positive PPD reactions, BCG vaccination history, or schools that explicitly accept chest X-ray as TB clearance.
Best for: students with prior positive PPD or BCG history.
SCHOOLS & COMPLIANCE PORTALS
CastleBranch, Complio, or Printed Form — We Sign Them All
Whatever upload portal your nursing program uses, the underlying physical form is similar. Bring whatever your school generated — printed PDF, screenshot, portal-specific form — and we’ll complete it during your visit.

FREQUENTLY ASKED QUESTIONS
Common Questions From Healthcare Students
Will Dr. Clay sign my CastleBranch or Complio form?
Yes. Bring whatever form your school generated — printed or on your phone — and we’ll complete it during your visit. We have not yet had a school form we couldn’t sign. If your school uses a portal upload, take a photo of the completed form with your phone and upload from the parking lot.
How fast can I get a same-day appointment?
Most healthcare students walk in without an appointment and are seen within 30 minutes. If you want a guaranteed time, book online — same-day appointments are typically available throughout the day.
What if my school requires Quantiferon specifically?
We offer Quantiferon-TB Gold at $400. It’s a single-visit blood draw with results in 24–48 hours. If you can’t make it back for a PPD read in 48–72 hours, Quantiferon is the better choice. Bring your school’s form — we’ll match the test to the requirement.
Do you draw titer blood for MMR, Varicella, or Hep B immunity?
We don’t draw titers in office. Most pharmacies and LabCorp Patient Service Centers around Carmel and Indianapolis can complete titer draws faster and at lower cost. Tell us which titers your school requires and we’ll point you to the closest convenient option.
Where can I get MMR, Varicella, or Hep B vaccines?
Most pharmacies in Carmel and Indianapolis carry these vaccines and administer them without an appointment. CVS, Walgreens, Kroger, and Meijer pharmacies all stock them. Bring your school’s vaccination requirement form — the pharmacy will document each vaccine in a format your school will accept.
Can I do my school physical, TB test, and Tdap in one visit?
Yes — if you choose Quantiferon or chest X-ray for the TB portion, everything happens in one visit and you walk out with completed paperwork. If you choose the two-step PPD, the only return is the 48–72 hour read — about 5 minutes, no appointment needed.
My school requires my five-year health history. Do I need records?
If you have your own copy, bring it. If not, we’ll ask the standard health-history questions during your visit and document what you tell us — that’s what most school forms actually require. Detailed records aren’t usually necessary unless you have a chronic condition that affects clinical work.
Do I need an appointment?
Walk-ins are welcome. If you’d like a guaranteed time, book online or call (317) 804-4203. Most students are in and out in under an hour with completed paperwork.
