INDIANA TB TESTING COMPLIANCE

How Monarch Medicine Helps You Meet Indiana's TB Testing Requirements

Indiana's TB testing requirements vary by industry — healthcare facilities, long-term care, home health agencies, and schools each have different rules. Whether you need a baseline test for a new hire or annual screening for your entire staff, Dr. Lisa Clay and the clinical team at Monarch Medicine handle the complete process so you stay compliant without the headaches.

Healthcare & Hospital Staff

We perform baseline two-step TST and IGRA (QuantiFERON Gold) testing for new hires, post-exposure testing after known TB contact, and annual symptom screenings for staff with a prior positive history. If a chest X-ray is needed, it's done on-site the same day — no hospital referral required.

Long-Term Care & Licensed Facility Staff

We administer two-step Mantoux baseline testing and ongoing annual screening to keep your facility in compliance with Indiana licensure requirements. Every test is documented with physician-signed results that satisfy IDOH record-keeping standards — we fax or email directly to your HR team.

Home & Community-Based Services (HCBS)

New for 2026: Indiana now requires all HCBS direct care employees to have a TB test upon hire, before any client contact. Monarch Medicine offers booked appointments testing 7 days a week so your new hires can get tested and cleared without delaying their start date.

Schools, Child Care & First Responders

We test school employees, licensed child care workers, EMS, fire, and law enforcement personnel. If your organization has specific documentation requirements, bring them in — we'll match the test type and paperwork to your compliance needs.

Individuals & Students

Need a TB test for a new job, college enrollment, or immigration physical? Book an appointment anytime — no employer account needed. Results and documentation are provided directly to you.

TESTING OPTIONS

Three Ways to Test — All Under One Roof

Monarch Medicine offers every standard TB test accepted by Indiana employers and regulatory bodies. Dr. Clay determines the appropriate test based on your history, employer requirements, and timeline.

PPD Skin Test (Mantoux)

The Standard Tuberculin Skin Test

How it works: A small amount of tuberculin purified protein derivative (PPD) is injected just under the skin of the forearm. You return in 48–72 hours so a licensed clinician can measure and interpret the reaction.

Turnaround: 48–72 hours (requires a return visit for reading)

Accepted for: Pre-employment screening, annual compliance, school enrollment, immigration physicals

1-Step & 2-Step available: We perform both single-step and two-step Mantoux tests as required by your employer or facility

Administered and read by TB-validated licensed clinicians

QuantiFERON Gold (IGRA Blood Test)

Single-Visit Blood Draw — No Return Trip

How it works: A standard blood draw is collected and sent to a reference lab. The interferon-gamma release assay (IGRA) measures your immune response to TB-specific proteins — no skin injection required.

Turnaround: 2–3 business days

Preferred for: Patients with prior BCG vaccination, those who cannot return for a 48–72 hour reading, healthcare workers requiring IGRA per facility policy

CDC recommended: The CDC considers IGRA the preferred test for most healthcare personnel screening

Only one visit required — ideal for busy employers

Chest X-Ray (Positive Result Follow-Up)

Required After Any Positive TB Test

How it works: If your PPD or QuantiFERON result is positive, Indiana regulations require a chest X-ray to rule out active pulmonary tuberculosis. Dr. Clay reviews the imaging and provides a physician evaluation and clearance letter.

Turnaround: Same-day imaging with physician review

Includes: Chest radiograph, physician interpretation, written clearance letter for your employer

No referral needed: Schedule — we handle the complete follow-up workflow in one location

Complete positive-result pathway without hospital referral

PHYSICIAN-SUPERVISED TB TESTING

Why Physician Oversight Matters for TB Testing

TB testing seems simple, but interpretation errors are common — and the consequences for employers and patients are serious. A false positive can cost an employee their job start date. A missed positive can put an entire facility at risk.

At Monarch Medicine, Dr. Lisa Clay, MD personally reviews every positive result, performs the chest X-ray evaluation, and writes the physician clearance letter. Unlike retail clinics where a technician administers the test and results are sent to an off-site provider, our entire testing and evaluation workflow happens under one roof with direct physician involvement.

"Employers trust us because we handle the entire process — the test, the reading, the X-ray if needed, and the clearance letter — all with physician oversight. Their employees don’t get bounced between multiple facilities." — Dr. Lisa Clay, MD, Owner & Medical Director

FREQUENTLY ASKED QUESTIONS

TB Testing — Frequently Asked Questions

Do I need an appointment for a TB test?

Yes. Monarch Medicine is walk-in friendly 7 days a week and for timing the test we ask you make an appointment.. You can also check in ahead of time through MyChart to save time.

What is the difference between a PPD skin test and a QuantiFERON blood test?

A PPD (Mantoux) skin test involves a small injection under the skin of the forearm. You must return in 48–72 hours for a trained clinician to read the result. A QuantiFERON Gold test is a single blood draw with no return visit — results are available in 2–3 business days. Both are accepted by Indiana employers. The CDC considers IGRA (QuantiFERON) the preferred test for most healthcare personnel.

Which TB test does my employer require?

It depends on your industry and facility type. Healthcare workers, long-term care staff, and HCBS employees each have different requirements under Indiana regulations. If you're unsure, bring your employer's documentation and Dr. Clay's team will help determine the right test.

What happens if my TB test is positive?

A positive TB test does not mean you have active tuberculosis. It means your body has been exposed to TB bacteria. Dr. Clay will order a chest X-ray — performed on-site the same day — to rule out active pulmonary TB. If the X-ray is clear, you'll receive a physician-signed clearance letter accepted by Indiana employers.

Do you provide clearance letters for employers?

Yes. Dr. Clay provides physician-signed documentation for all TB test results. We can fax or email clearance letters directly to your employer on request.

What is the new HCBS TB testing requirement for 2026?

As of January 20, 2026, all direct care employees in Indiana's Home and Community-Based Services (HCBS) programs must have a TB test upon hire, before any face-to-face interaction with clients. Documentation must be maintained in the employee's personnel file.

Do I need a two-step TB test?

Two-step Mantoux testing is required for baseline screening at many licensed facilities in Indiana, including long-term care and nursing homes. The second step is administered 1–3 weeks after the first. Monarch Medicine performs both steps. If your employer requires a two-step test, plan for two visits.

How much does a TB test cost?

All TB tests at Monarch Medicine are self-pay with transparent pricing and no hidden fees. See our pricing section above for current rates, or call (317) 804-4203.

Can I get a TB test on the weekend?

Yes. Monarch Medicine is open Saturday and Sunday from 9:00 AM to 12:00 PM. Walk-ins are welcome appointment required for TB testing, due to timing issues. Note that if you receive a PPD skin test on Saturday, your reading visit will fall on Monday or Tuesday.

I had a BCG vaccine as a child. Which test should I get?

The QuantiFERON Gold blood test is preferred for patients with a history of BCG vaccination. The PPD skin test can produce a false positive in BCG-vaccinated individuals, while the QuantiFERON test is not affected by prior BCG vaccination.

PATIENT REVIEWS

What Our Patients Say

★★★★★

"The staff were incredibly friendly, attentive, and genuinely caring when I brought my daughter in. They created a welcoming environment with coffee, water, and snacks in the waiting area, which was a thoughtful touch. I also really appreciated their transparent pricing. They bill insurance but also offer very reasonable cash-pay options, which provides peace of mind compared to the unpredictable costs of an ER visit. Most importantly, the experience felt personal rather than rushed. Highly recommend if you’re looking for compassionate, patient-centered care."

—Brittany Carter

★★★★★

"Dr. Clay is great, always nice and makes sure I’m up to date on any questions I may have. Nurse Olivia is always happy and smiling. It’s a great place!!!"

—Greg Newcomb

"Going to this Urgent Care is like visiting my own doctor…not your typical urgent care facility. This is my second visit in less than a year upon someone’s recommendation. Virtually no wait and Dr Clay really listened and got to the root of my problem. Highly recommend!"

TB testing at Monarch Medicine is performed for screening and compliance purposes. A positive TB test does not confirm active tuberculosis — additional evaluation including chest X-ray and physician assessment is required. All testing is administered by licensed clinical staff under physician supervision. Results are interpreted according to CDC and Indiana Department of Health guidelines. This page is for informational purposes and does not constitute medical advice. Patients with symptoms of active TB (persistent cough, fever, night sweats, unexplained weight loss) should seek immediate medical evaluation.
