Our AfterCare team, consisting of dedicated doctors and clinical assistants, is here to facilitate the next steps on your path back to health. We are committed to ensuring that you receive continuous support and comprehensive care even after you leave our facility.  With MyChart, you will have access to the After Visit Summary (AVS), Lab, Imagine Results, and much more.

Our team of doctors and clinical assistants is dedicated to serving Monarch Medicine patients' post-visit care needs. We provide a range of aftercare services to ensure a smooth and effective recovery process:

• Relaying Lab and Radiology Imaging Results: We communicate directly with you about your lab and imaging results, ensuring you understand your health status and any necessary follow-up actions.  In addition to this we provide MyChart, giving you convenient access to your chart.
• Assisting with Specialist Appointments: We coordinate referrals and schedule follow-up appointments with specialists, making sure you receive the specialized care you need.
• Answering Your Care Questions: Our team is available to answer any questions you may have about your care, providing clarity and peace of mind.
• Completing Patient Forms: We assist in completing necessary patient forms, ensuring all documentation is accurate and timely.
• Generating Work Notes: We provide work notes for your employer as needed, supporting you in managing your health and professional responsibilities.
• E-Prescribe: A convenient service that allows us to send your prescriptions directly to your pharmacy electronically, ensuring a seamless and efficient experience for our patients.
• Follow Ups:  We ensure that a follow-up appointment is always available if it's needed as part of your care. We are committed to supporting your recovery journey and are here for any further care or concerns you may have .
